Creatine: The Science Is Settled. Here's What It Means for You.

It’s hard to ignore the hype around creatine: If the algorithm has even a whiff of evidence that you have an interest in fitness, you've been bombarded with promises about it. And yet the benefits of creatine for muscle building have been well established for years. Personally, I've been happy with my physique, so I never tried taking creatine. It wasn't until late spring of 2025 that the sheer volume of noise about creatine finally got me to try it myself. Three weeks in, my stamina climbed noticeably, especially during lifts and hard sprints. But the real win was how much less sore I was after hard workouts. This was an absolute game changer!
Creatine is one of the most studied supplements out there, and the evidence for its benefits is about as solid as sports nutrition gets. Here's what the research actually says, plus how to use it well:
What Creatine Actually Does
Creatine helps your muscles regenerate ATP, the energy source for short, high-intensity efforts such as heavy lifts or sprints. More available energy means you can push a rep or two further, recover faster between sets, and accumulate more total training volume over time. More volume, over weeks and months, is what drives strength and muscle growth.
Proven Efficacy for Weightlifting: 3-5g a Day
The gold-standard dose for strength training is a simple 3-5 grams a day, taken consistently. You don't need to cycle it, and you don't need a "loading phase" (a short period of much higher doses) unless you're in a hurry to saturate your muscle stores. A steady 3-5g daily gets you to the same place in about three to four weeks.
At this dose, creatine is consistently shown to be the single most effective legal supplement for increasing high-intensity exercise capacity and lean mass when paired with resistance training. Two effects stand out in particular:
Increased effort and output. With more readily available cellular energy, you can push harder in the moments that matter most, pushing out one last rep. This compounds into more total training stimulus over time.
Decreased DOMS. Some research suggests creatine may reduce muscle damage markers and ease delayed onset muscle soreness (DOMS) after hard training, likely by improving cell hydration and reducing inflammatory response. Like myself, several of my clients have noticed a significant decrease in DOMS.
Is It Safe?
Yes — this is the part where 25+ years of research gives a clear answer. In healthy adults taking recommended doses, clinical trials have not found evidence of harm to the kidneys, liver, or muscles. Creatine monohydrate is considered one of the safest and most well-studied supplements available.
The one caveat worth knowing: creatine supplementation can cause a small, temporary rise in serum creatinine — a lab marker doctors use to estimate kidney function. This isn't actual kidney damage; it's a byproduct of increased creatine turnover. But if your doctor sees an elevated creatinine number without knowing you supplement, it could look like a red flag when it isn't.
The Cognitive Frontier: Promising, But Early
This is where things get interesting — and where we should be honest about what we don't yet know.
A growing body of research is looking at creatine's effects on the brain: memory, attention, processing speed, and resilience under stress like sleep deprivation. Recent meta-analyses have found real, measurable improvements in memory and cognitive performance with supplementation. There's even early work exploring creatine's role in conditions like depression and Alzheimer's.
It's genuinely exciting. But it's still early-stage research, and the science is evolving fast — treat this as promising, not proven.
One key wrinkle: your muscles are greedy. They soak up creatine first, and 5g a day is largely spoken for by muscle tissue. Getting enough left over to meaningfully raise brain creatine levels appears to take a larger dose — researchers are looking at around 10 grams a day for potential cognitive benefits. That's double the standard strength-training dose, and the "optimal" cognitive dose is still not firmly established.
Do's and Don'ts
Do:
• Drink more water. Creatine pulls water into your muscle cells, so your overall hydration needs go up.
• Start slow. If you're new to it, try 2.5g at a time with food for a week or two rather than jumping straight to a full dose to help minimize digestive upset and excessive water retention while your body adjusts.
• Tell your doctor you're supplementing. Always discuss a new supplement with your doctor. This is especially true for creatine because it can raise creatinine levels on bloodwork.
• Take it consistently: every single day of the week, including rest days. Creatine isn't a same-day performance boost; it works by building up in your muscle stores over time, so skipping days slows that saturation down.
• Choose creatine monohydrate specifically and look for third-party tested products (NSF Certified for Sport or Informed Sport). It's the most commonly studied form, backed by 500+ peer-reviewed studies (versus well under 20 for newer forms like creatine HCl).
• Pair it with resistance training. Creatine amplifies a training stimulus — it isn't one on its own.
Don't:
• Start if you already struggle to drink enough water. Creatine adds to your hydration demands, so if plain water intake is already a challenge for you, that's worth solving first.
• Panic about the scale. A small weight increase up front is normal and expected — it's water being pulled into your muscles, not fat gain. Over three months, studies show creatine supplementation alongside weightlifting can increase lean mass by roughly 1-1.5 kg (2.2–3.3 lbs).
• Expect overnight results. Muscle creatine stores build up over days to weeks. This isn't a pre-workout jolt, it's a cumulative effect.
• Self-experiment if you have kidney disease, are pregnant or breastfeeding, or are taking medications that affect kidney function. Talk to your doctor first. The safety data is based on healthy adults. (For what it's worth, creatine at normal doses isn't linked to kidney stones in healthy people. The risk factors that matter are dehydration, very high doses, and pre-existing renal issues — not creatine itself.)
